How to get your sports physical done in one visit

August is our busiest month for school and sports forms. A little preparation gets you in and out.

What to bring

The form itself from the school or league, your insurance card and ID, a list of medications, and your immunization record if you have one. Parents need to sign the consent section for anyone under eighteen.

What we check

Height, weight, blood pressure, vision, heart and lungs, joints and flexibility, and a short conversation about any past injuries, fainting, chest pain or family heart history.

If something comes up

Occasionally we find something worth investigating before clearance — usually a heart murmur or a blood pressure reading. We will explain what it means and arrange the next step quickly so it does not cost your child a season.
