What to expect

1Bring your form — Whatever the school, league or employer gave you — we'll complete it during the visit.
2Vitals and history — Height, weight, blood pressure, vision, and a review of medications and past problems.
3Exam — A head-to-toe check appropriate to the form, including heart and lungs for sports clearance.
4Signed and done — You leave with the completed paperwork, plus a copy for your records.

When to go to the emergency room instead

Chest pain, trouble breathing, heavy bleeding, sudden weakness or confusion, a serious head injury, or any symptom that feels life-threatening — call 911 or go to the nearest ER. We'd rather you be safe.
